These forms are required by the first day of the school year Pennsylvania Department of Health requires a Physical exam and Dental exam for certain grades Kindergarten (Kdg) New First Grade (1st) - Physical Exam and Dental Exam must be done after Sept. 1 of the year before entering Kindergarten or First Grade. Third Grade (3rd) - Dental Exam (done anytime after Sept. 1 of second grade) Sixth Grade (6th) - Physical Exam (done anytime after Sept. 1 of fifth grade) Seventh Grade (7th) - Dental Exam (done anytime after Sept. 1 of sixth grade) Tenth Grade (10th) - Physical Exam ( done anytime after Sept. 1 of ninth grade) NEW STUDENTS in all grades are required to have a current Physical Exam and Dental Exam on file.
