MEMORANDUM
Annual Health Screening Information
Dear Parent/Guardian:
Annually, Council Rock School District school nurses, perform general screening procedures for each student. These may include height, weight, BMI (body mass index) calculation, vision and hearing. When results of vision and hearing screenings are outside normal limits parents are notified.
Height, Weight, BMI calculation and Vision Screening are performed annually.
Hearing Screening is performed annually for students in grades K, 1, 2, 3, 7, 11 and for other students when there are hearing concerns.
Each year this information is recorded in your child’s health record and is available to parents or guardians. Due to confidentiality of the screening results, CRSD must provide these results directly to you.
A district wide mailing of these results to your home is very expensive, and in this tough economic climate we are looking at ways to minimize or reduce these costs. This savings can hopefully be used productively for important instructional programs that benefit your child.
Please contact the health office at Council Rock High School South at 215-944-1109 and set up a time to come in and pick up your child’s health report card or request email notification so the cost of mailing may be eliminated. The health office is open from 7:25-2:55.
