As of 6/1/2017 you must login to ATS to complete the pre-participation packet. This includes recertification for the winter and spring season. It is VERY easy to complete on-line.

Return to activity form
If an athlete has been seen by a physician for an injury the physician must complete this form before the athlete will be allowed to return to activity. This is a one page form.

We are pleased to announce that starting in the 2016-2017 school year we have started using a new software program called ATS. In ATS you can complete all physical paperwork except Section 6, the physician page. You can print out and upload Section 6 into ATS eliminating the need to send in paper forms.
Every student has been assigned a login and password. To login and complete the student paperwork goto:
