Letter of Recommendation

If you'd like a letter of recommendation from me, or any forms filled out as a reference, fill out the attached form (below) and get it back to me via e-mail (as an attachment). 

I normally acknowledge receipt of your letter almost immediately. If you don't hear back from me in a day, assume something went wrong and I didn't get it yet.
