Wellness Survey

As part of the district’s continued efforts to ensure we are addressing all areas of need for our students, we are asking all Council Rock families and staff members to complete the following survey: Council Rock Student Wellness Survey - Formstack.

The survey will be open for responses until midnight on Sunday, November 10. 

The feedback you provide in this survey will be used as an additional piece of data to help guide the district in determining which, if any, additional types of support in the areas of emotionality, social skills, and behavior would be helpful for our students. The survey should take approximately five minutes. We are very grateful for your time and input!
